Privacy + Cancellation

 

Privacy Statement

Tribe Disability Support complies with all relevant privacy legislation and has systems for the collection, use, disclosure, quality, security, accuracy, and correction of personal information relating to you, as our participant.

Your privacy and dignity will always be maintained. You will be asked to sign a Privacy Agreement to consent to collect, use, and disclose your personal information to comply with the Privacy Act 1988. If you would like more information about this, you should read our Privacy and Confidentiality Policy and Procedure. Information regarding your privacy is also included in your service agreement.

Requests for access to the personal information we hold should be made in writing to the Manager. Where you believe that a breach of this policy or the Privacy Act has occurred, a written complaint should be made to the Manager. Failure to receive a response within thirty (30) days, or you are dissatisfied with the response, you may complain to the Office of the Australian Information Commissioner (OAIC) via:

Cancellation

We know that things can change and if you need to cancel your support from Tribe Support at any time, you agree to give us as much prior notice as possible. A cancellation is a short notice cancellation (no show) if:

  1. You do not show up for scheduled support within a reasonable time or are not present at the agreed place and within a reasonable time when Tribe Support is traveling to deliver support.

  2. Less than 24 hours’ notice for any other support. If support is canceled by the participant or participant representative within 24 hours of the support being delivered, Tribe Disability Support is able to charge the participant’s NDIS funding for the entire duration of the support that was to be delivered.
